

The admissions process usually begins with a conversation with our social worker, or one of the Little Sisters, during which we will seek to ascertain the following:
- The age and financial eligibility of the prospective Resident
- Their current living situation
- Their needs and health concerns
- Hopes and expectations of the individual and their family regarding placement in a long-term care facility
These questions help us to discern if the individual’s needs and desires would be met through the services of the Sacred Heart Home. We work with the applicant and their family members and are open to visits to the Home before any final decision on admission is made. Our goal is to make sure that admission to the Home is a good fit for the person involved.
You should know that we do not generally admit directly into the skilled nursing unit. These rooms are usually reserved for Residents in the assisted living or apartment areas who require a higher level of care. If we are not able to meet your needs, we will suggest other facilities and resources in the community which may be helpful to you.
